NEW MEDICAL OFFICE CHANGES!
Starting October 1st, 2024, our clinic will adopt a new Patient Enrolment Model called a Family Health Organization (FHO). Our group, called the "Aurora-Summerview FHO," consists of 7 family physicians and their enrolled patients. You are receiving this letter because you are rostered with one of our physicians.
The FHO is important because physicians are required to provide high-quality primary health care services to enrolled patients, including after-hours access.

Key Changes to Note:

FHO Physicians Only:
​You must receive family doctor/primary care services only from your family physician and urgent care from one of the 7 FHO physicians working in our clinic. (Services from outside physicians are not permitted and considered "Outside Use.")
Why Choose the Family Health Organization (FHO)?
When you enroll with a FHO physician:

You become part of that doctor’s roster.

The physician's salary is based on the number of enrolled patients and their demographic characteristics (age, sex, etc.), rather than being paid per individual visit or service.

Same-Day Appointments:
​We can offer same-day appointments for urgent needs.

Convenient Problem Resolution:
​Many simple issues can be addressed over the phone or through my staff, reducing unnecessary office visits.

More Time for Important Tasks:
With fewer unnecessary visits, I can dedicate more time to:

Advocating for the tests or services you need.

Making appropriate referrals.

Completing forms and writing letters on your behalf.

Returning your phone calls promptly.

​After-Hours Clinic Access:
For urgent problems after office hours, FHO patients can access the After Hours Clinic:

Monday to Thursday: 5 pm to 8 pm

Saturday: 10 am to 2 pm

Access to a Doctor When I Am Away:
You will have access to another doctor in the clinic for urgent issues when I am unavailable (e.g., during vacations).

Preventative Care Programs:
As part of the FHO, you will receive reminders for important screening tests, such as:​

Mammograms

PAP smears
-
Fecal occult blood tests for colorectal cancer

Always Call Us First:

Avoid "Outside Use":
In the future, if the Ministry of Health informs us that you’ve used another clinic or primary care physician without our referral, my staff will reach out to discuss the situation.

Let Us Know if FHO Doesn't Work for You:
If you cannot meet the terms of the FHO agreement (e.g., if you live far from the clinic), let us know. We may need to adjust your enrolment.
